Job Description

The Registered Nurse functions as a caregiver for a specific patient population to achieve desired outcomes. This role involves expanding knowledge and skills, providing effective direct care as part of an interdisciplinary team to complex patients. The nurse oversees care provided by others through delegation, validation, evaluation, and coordination, ensuring seamless patient care among nursing peers, physicians, and support services.

Responsibilities
  • Deliver nursing practices based on evidence-based research, policies, and standards of care to ensure safe, individualized patient care.
  • Collect comprehensive data relevant to the patient's health, performing age- and disease-specific assessments independently.
  • Recognize clinical and diagnostic status changes promptly.
  • Identify educational needs based on age, disease, and cultural differences.
  • Develop and coordinate a patient-centered plan of care with clear goals and outcomes.
  • Communicate and implement the care plan effectively among team members.
  • Provide ongoing communication and education to patients and families, ensuring understanding.
  • Document assessments, care plans, interventions, and evaluations accurately using electronic systems.
  • Administer medications safely utilizing advanced bar-scanning and documentation technologies.
  • Coordinate multidisciplinary discharges for safe patient transitions.
  • Evaluate progress towards outcomes and adjust interventions as needed.
  • Respond to changes in patient condition with appropriate interventions.
  • Identify and address ethical issues in practice.
  • Enhance patient satisfaction through proactive care and communication.
  • Maintain required certifications and continuing education to meet accreditation and licensing standards.
  • Adhere to the American Nurses Association Code of Ethics.
  • Perform physical tasks including lifting, pushing, pulling, and standing for designated periods.
  • Comply with organizational standards and behaviors.
  • Perform other duties as assigned.
Qualifications

Required:

  • Associate Degree in Nursing or Nursing Diploma.
  • Progress toward or completion of a BSN within three years of hire.
  • Minimum six months relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
  • Ability to learn quickly, adapt, and demonstrate accountability and teamwork.

Preferred:

  • National Certification in area of specialty when eligible.

Licenses and Certifications:

  • NJ State Professional Registered Nurse License.
  • BLS/CPR certification from the American Heart Association within 60 days of hire.
