Job Description Summary:

Responsible for telephonic management of patients at the telephonic center.

Job Description:

Job Responsibilities:

  • Responsible for assigned group of patients including assessments of patients’ initial needs, coordination of all services of patients with Option Care Health, evaluation of services to be rendered to individual patients, promotion of physician/client and patient satisfaction, and communication of all aspects of patient care to the Clinical Director. This involves coordination of care from referral to dispatch of homecare nurses, as well as coordination of care with respect to telephonic management and interfacing with patient’s physician.
  • Provide patient education through utilization of nursing assessment process. This involves the specific educational needs of the high-risk obstetrical patient during the antepartum period. Communication of educational material to the patient and their support system may be through any of all of the following modalities: verbal, written, computer-based, etc.
  • Collaborative work with patient’s health care team through nursing assessment, patient education, needs planning, and evaluation of outcomes through follow-up communication with patient and physician/client offices.
  • Incumbent to our services will notify patient’s physician for aberrancies in normal assessments or deviations from physician-determined thresholds.
  • Execution of physician orders regarding patient care that relate to telephonic nursing support and management.
  • Provide excellent communication to nurse colleagues regarding patients on service with review of current status, status of physician orders, any other issues regarding insurance authorization or pending patient issues. This is also a function of coordination of care.
  • Documentation either written or computer based of all services, educational efforts, communications with patient, family or physician and/or physician office personnel.
  • Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned.

Supervisory Responsibilities : i.e. hiring, recommending/approving promotions and pay increases, scheduling, performance reviews, discipline, etc.

Basic Education and/or Experience Requirements:

  • Graduate of an accredited school of nursing
  • Current Registered Nurse (RN) licensure in the state of practice
  • 5 years of recent clinical experience with labor & delivery, high-risk antepartum nursing care and/or perinatal home care

Basic Qualifications & Interests:

  • Ability to prioritize case load requirements and superior organizational skills
  • Ability to use computer/word processor for interpretation of patient data and recording of information on computerized charting system
  • Excellent clinical assessment skills and ability to formulate educational plan for patient
  • Excellent clinical interpretation skills
  • Appropriate telephone skills and manners
  • Excellent oral and written communication skills
  • Ability to work independently within the team framework
  • Flexibility in personal work schedule with ability to provide call-coverage from home
  • Access to reliable, high-speed internet and quiet work environment for remote placement

Physical Demand Requirements

  • Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required by this job include ability to see up close and from a distance, color and peripheral vision, depth perception and the ability to adjust focus.
  • Team members in this job classification have the likelihood of occupational exposure to blood, body fluids and other potentially infectious materials. Possible exposure to hazardous substances with possible effect on reproduction, injury from needles, other sharps, fumes, chemicals, humidity, cold, heat, adverse weather elements, animals, secondhand smoke/vape and unpredictable home environments.
  • Safety requirements include closed toe-shoes, facemask, goggles, gown and/or gloves and functioning device for communication in both routine and emergency situations.

Preferred Qualifications & Interests:

  • One year of experience in Home Health provision
  • Spanish-speaking

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.

Pay Range is $39.98-$66.64
