Job summary

Trinity Health IHA Medical Group in Ann Arbor is seeking an experienced Registered Nurse (RN) to join our outpatient care team. The RN III role involves advanced training, leadership, care coordination, and patient education to improve outcomes.

You will work with providers, care managers, and other specialists, handling triage by phone and in-office visits, managing referrals, and supporting transitions of care. BSN strongly preferred; Michigan RN license required.

Qualifications

• BSN strongly preferred or graduate of an accredited RN program.
• Advanced certification/training relevant to specialty (e.g., AAACN, IBCLC) or graduate degree preferred.
• 3 years of clinical experience in ambulatory care or specialty is expected.

Responsibilities

• Proficient to work within scope of nursing certification including lactation consulting, advanced wound ostomy care, care/case management certification, diabetes educator or asthma educator certification.
• Serve as a nurse leader by assisting management with hiring, motivating and engaging clinical staff.
• Identify and target at-risk patients requiring extensive care to improve health outcomes.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.

Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Registered Nurse (RN) to join our outpatient care team. In this role, you'll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience. The RN III works in a capacity that requires advanced training, education, certification and/or leadership. Additionally, RN III's may have program administration and staffing development responsibilities.

What You'll Do:

What We're Looking For:

EDUCATION: BSN strongly preferred or Graduate of an accredited Registered Nurse program. Advanced certification/training relevant to specialty (e.g., AAACN, IBCLC) or graduate degree preferred.

CREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan. Valid CPR certification.

MINIMUM EXPERIENCE: 3 years of clinical experience in a ambulatory care or specialty, and/or demonstrated competency in IHA's RN II position.
