Job summary

Mission Neighborhood Health Center in San Francisco seeks a Registered Nurse - Disease Management who is bilingual in English and Spanish. The role supports patient care across multiple locations, coordinates clinical work, and participates in telehealth visits and discharge planning.

You will work under the Senior Nurse Manager in an exempt position, with an hourly starting range of $47.32–$50.95, and provide continuity of care, education, and prevention services to diverse communities served

Qualifications

  • Valid California Registered Nurse License.
  • Bilingual in English and Spanish.
  • Two years of experience in ambulatory care clinic or hospital experience.

Responsibilities

  • Coordinates patient flow in clinical area.
  • Obtains and records accurate medical information and history (charts, forms, prescriptions, labs, X-ray requests).
  • Performs assessments and prepares patients for care based on condition.
  • Provides medical treatment and procedures per MNHC nursing protocol.
  • Administers medications as ordered.
  • Conducts triage, emergency screening, and patient education.
  • Provides telehealth visits and discharge planning; attends staff meetings, maintains patient confidentiality.

We are currently looking for qualified candidates to join MNHC as a Registered Nurse - Disease Management.Please note: This role requires applicants to be bilingual in English and Spanish.This position reports to theSenior Nurse Managerand is Exempt role with astarting hourly range of $47.32 - $50.95 (depending on years of clinical experience post RN license)

Primary Objective:

The Registered Nurse is responsible for the delivery of quality nursing care and will assist in establishing a clinical team approach to comprehensive medical care and provide continuity of care to MNHC patients at multiple locations.

Essential Functions/Responsibilities:
  • Coordinates patient flow in clinical area.
  • Responsible for obtaining and recording accurate medical information and history. This includes the following:
    • Checking patient charts
    • Checking encounter forms, prescriptions, lab and X‑ray requests.
    • Completing disability, CHPD, and other medically related‑ forms.
    • Maintaining tracking systems and follow-up reports/requests.
    • Initiating health maintenance and prevention procedures.
  • Assesses initial health needs of patients and prepares them as indicated or as condition warrants.
  • Provides medical treatment as ordered and performs procedures according to MNHC nursing protocol.
  • Prepares and administers medication as ordered.
  • Responsible for triage/emergency and medical screening of patients in charge area.
  • Provides patient education and counseling.
  • Provides telehealth visits, including hospitalization pre and post discharge planning
  • Attends and participates in nursing staff meetings as needed.
  • Maintains confidentiality of all patient information according to federal guidelines and regulations.

  • Valid California Registered Nurse License.
