Saint Regis Mohawk Tribe Health Services seeks an RN/Care Coordinator to support mental health therapists, psychiatrists, and the outpatient ACDP staff. You will triage referrals, provide injections, assist with case management, and link clients to medical and behavioral health services.
Strong communication and computer skills are essential. Licensed in NY with at least one year of medical-surgical RN experience; behavioral health experience preferred.
GENERAL SUMMARY: Under the direction of the Psychiatric Nurse Practitioner, the RN/Care Coordinator will assist the Mental Health therapists, Psychiatrist, Psychiatric Nurse Practitioner, and the Outpatient Alcohol and Chemical Dependency Program staff with meeting the client's needs
The RN/Care Coordinator will work within guidelines of the Saint Regis Mohawk Tribe Health Services policies and procedures, including Mental Health and A/CDP, and under standards of care according to New York State Office of Mental Health and New York State Office of Substance Abuse and Alcohol Services.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Mandated Reporter: Must become a mandated reporter prior to having contact with children in a job-related capacityand maintain that status throughout employment. This includes completing any necessary training and adhering to all reporting obligations as defined by applicable laws and tribal policies.

EDUCATION and/or EXPERIENCE:
AND 1 year medical-surgical hospital experience as an RN.
OTHER SKILLS and ABILITIES:Familiarity with Akwesasne Community. Interest in obtaining additional training/education in Behavioral Health. Strong oral and written communication skills required. Must have excellent computer skills including word, excel, and experience with varying databases. Computer testing in the applicable skills may be a requirement during the interview process. Openness in working with traditional/spiritual approaches regarding behavioral health issues. Ability to do presentations, represent the mental health program and connect with people. Motivated, caring and compassionate, committed to suicide and substance use prevention, reducing impact of stigma and increasing help seeking behavior. CPR certification and physically able to carry out the duties of the position.
REASONING ABILITY:Maturity and ability to handle delicate role issues and to respond to crisis.Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
