Champion City Nursing & Rehabilitation, located in Pittsburgh, is seeking an MDS Nurse to lead assessment coordination and care planning for residents. In this role, you will develop individualized care plans, perform comprehensive assessments, and ensure adherence to MDS and other regulatory guidelines across facilities.

Qualified candidates hold RN certification and have strong organizational and communication skills; MDS coordination experience is preferred.

About the Role:
