What You Would Bring:
️ High School Diploma or G.E.D
️ Current RN License
️ RNAC Certification
️ Completion of Infection Control Training
️ Strong computer proficiency and analytical skills
Key Responsibilities Include:
• Coordinate the facility’s Resident Assessment Instrument (RAI) process per federal and state guidelines
• Accurately complete MDS assessments and ensure proper clinical documentation
• Develop and continually evaluate residents’ comprehensive care plans
• Lead interdisciplinary Utilization Review meetings
• Audit medical records and provide staff education regarding MDS coding
• Collaborate with physicians, health plans, and the care team for approvals and coverage
• Serve as a subject-matter expert on SNF payment systems and quality reporting
Physical Requirements:
• Ability to move intermittently throughout the workday
• Capable of assisting in emergency resident evacuations
• Must be able to bend, kneel, crouch, and lift overhead (up to 50 lbs) as needed
• Communicate effectively with medical, nursing, and interdisciplinary teams
