Responsibilities

• Conduct MDS and other admission assessments to identify care needs and develop individualized care plans.
• Collaborate with interdisciplinary team members to ensure effective care delivery and coordination.
• Monitor and evaluate resident outcomes, adjusting care plans as necessary.
• Maintain accurate and timely records of resident assessments, care plans, and outcomes.
• Participate in quality improvement initiatives to enhance care and services.
• Will be part of the on-call rotation with weekends as needed.

Job Summary:

We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.
