Job summary

Community Health Network seeks an experienced Registered Nurse for Ambulatory Triage in its Internal Medicine Care team in Kokomo, IN. The role includes phone triage, patient education, scheduling, prior authorizations, prescription refills, referrals, and comprehensive care coordination, all performed on-site.

Ideal candidates have an Indiana RN license, 3+ years of nursing experience, and outpatient experience.

Qualifications

• RN license in Indiana (NLC) required.
• Graduate of NLN/CCNE/ACEN or NLN CNEA accredited nursing program or equivalent experience.
• Three years or more of nursing experience required.
• Previous medical office/outpatient experience preferred.
• Excellent team skills.
• Compassionate and caring; accountable and self-directed.

Responsibilities

• Conduct phone triage and patient education.
• Schedule appointments and coordinate care.
• Handle prior authorizations and prescription refills.
• Manage referrals and care coordination tasks.
