• Develop criteria for measurement of patient outcomes and establish individualized patient goals.
• Formulate and document the plan of care, coordinating with physicians and health team members.
• Implement the plan of care while ensuring safety precautions and providing emotional support.
• Evaluate care provided and document effectiveness, performing variance analysis for performance improvement.
• Collaborate with care team members and assist physicians during treatments and diagnostic procedures.
• Provide wound care and care for pressure ulcers (Stages I-IV).
• Administer medications correctly and educate patients on drug interactions.
• Participate in patient and family education, coordinating referrals for follow-up care.
