Job Overview

We are committed to patient- and family-centered care. The professional nurse utilizes the nursing process to address the physical, psychological, emotional, and social needs of our diverse patient population. As a vital part of the primary care team, the nurse collaborates with providers and support staff, delivering clinical interventions and communicating effectively with patients to meet their individual needs and respect their cultural beliefs. A current nursing license in the state of practice is required.

Key Responsibilities

• Integrate evidence-based practice, clinical expertise, and patient preferences for optimal healthcare delivery.
• Collaborate with primary care providers and support timely communication with specialists.
• Support clinical care pathways and participate in quality improvement initiatives.
• Implement ambulatory care nursing interventions during various patient interactions.
• Educate patients and families on preventive care, chronic disease management, and medication use.
• Accurately document all clinical interactions in the electronic medical record (EMR).
• Coordinate care activities such as managing abnormal results, triage, vaccinations, and in-basket management.
• Ensure safety and quality of care by following established protocols.
• Utilize technology and data to monitor care outcomes and support continuous improvement.
• Work effectively within inter-professional teams, fostering respect and shared decision-making.
