Southern New Hampshire Health is seeking a per diem Registered Nurse to support a multi-site practice network in New Hampshire. The role involves nursing care across in-person and telehealth settings, with focus on patient assessment, care planning, and interdisciplinary collaboration.

Responsabilités

• Carry out the nursing process in person, by telephone (triage), and via telehealth.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect specimens as per state Nurse Practice Act.
• Educate patients on self-management activities, medications, nutrition, health, and wellness.
