Job summary

Agape Care Group in Louisiana is seeking an After-Hours Hospice RN to join our on-call team from 5pm Friday to 8am Monday. You will provide compassionate, patient-centered nursing care to our hospice patients and coordinate with families and the care team to uphold quality of life.

This full-time role offers supportive leadership, opportunities for professional development, and a strong work-life balance in a community where care for others is a calling.

Qualifications

• Active Louisiana RN license.
• 2+ years nursing experience, hospice experience preferred.
• Ability to lift 80–100 lbs and perform physical tasks.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide after-hours on-call nursing care from 5pm Friday to 8am Monday.
• Act as patient advocate and coordinate plan of care with patient, family and hospice team.
• Deliver compassionate end-of-life care and ensure quality care for patients and families.

As a registered nurse, you’ll be a voice for your patients, all while communicating with everyone involved in the plan of care—the patient, the patient’s family, and the hospice care team. You’ll serve as the patient advocate being able to ensure every patient receives the quality care they deserve. After-Hours Hospice RNs are salaried full‑time positions to cover on call after hours from 5pm on Friday to 8am Monday morning. You will be fully supported by our 24/7 RN staffed Triage Department alongside your local hospice team.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
