Job summary

Bristol Hospice is seeking an Admissions RN to establish and nurture relationships with customers and referral sources in the community, opening access to Bristol Hospice Services in the areas we serve. The role involves coordinating referrals, admissions, and initial patient assessments to ensure timely and compassionate hospice care.

You will work with the DPCS, ED, and Community Education Director, conduct community education, participate in QA/QI as needed, travel up to 20%, and maintain

Qualifications

  • Must have RN license in the state.
  • Minimum 1 year of professional nursing experience within the last 3 years or a BSN from an NLN-accredited program.
  • Willingness to travel 20% of the time.
  • Flexible in work hours.
  • Willingness to maintain knowledge of information systems and applicable software programs.

Responsibilities

  • Relate information to DPCS, ED and Community Education Director to promote hospice access.
  • Assess community educational needs related to hospice care.
  • Establish and monitor perceptions of Bristol Hospice as a high-quality provider.
  • Facilitate communication with agency and referral sources to promote continuum of care.
  • Maintain knowledge of hospice educational resources and share with staff.
  • Facilitate the hospice referral process.
  • Review patient information to determine hospice needs and criteria.
  • Coordinate with leadership and referral sources for timely admissions.
  • Complete hospice consults/evaluations and admissions.
  • Complete initial patient and family assessments.
  • Facilitate and complete admissions as indicated.
  • Provide comprehensive physical assessments and history.
