Job summary

Bristol Hospice seeks an Admissions Nurse to establish relationships with customers and referral sources in the community. You will develop access to Bristol Hospice Services in the service area and coordinate admissions with facilities, physicians, hospitals and hospice programs.

Responsibilities include completing admissions, educating patients and families, participating in IDT meetings, and ensuring timely, quality care planning. Travel about 20% is expected.

Qualifications

  • Must have an RN license in the state.
  • Minimum of one year of nursing experience in the last three years or a BSN from an NLN-accredited program.
  • Ability to travel about 20% of the time, often with little notice.
  • Excellent communication and organizational skills; plan of care documentation proficiency.

Responsibilities

  • Relate information to DPCS, ED and Community Education Director to promote hospice access.
  • Assess and meet community education needs related to hospice care on an ongoing basis.
  • Improve perceptions of Bristol Hospice as a high-quality provider in the community.
  • Coordinate referrals/admissions with facility, physicians, hospitals, SNFs, and hospice programs.
  • Complete hospice consults, evaluations and admissions.
  • Provide patient/family education per care plan.
  • Participate in IDT meetings and contribute to the plan of care as needed.
  • Assist intake as liaison between facilities and hospice services.
