Elara Caring in Mexico, MO is seeking a Registered Nurse (RN) Home Health to deliver high-quality in-home care. You’ll assess patients, plan and implement care, and coordinate with an interdisciplinary team.

Requirements include an active RN license, 1+ year clinical experience, willingness to travel up to 50%, and the ability to lift up to 50 lbs. A nursing degree (ADN/BSN) is preferred; HomeCare HomeBase experience is a plus.

Qualifications

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Renal care/dialysis experience preferred.
• Willing to travel within branch coverage area (up to 50%).
• Must have a dependable vehicle and valid driver's license.

Responsabilités

• Assess patient needs and plan, coordinate, implement, and evaluate the plan of care.
• Perform initial evaluations and follow-up visits per patient/family needs.
• Communicate findings to Clinical Supervisor and care team.
• Adjust plan of care in response to patient changes and notify the team.

Makes follow up visits according to patient, family, and facility needs.* Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.* Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.
