Job summary

Amedisys is seeking a Registered Nurse for a 13-week assignment in Roanoke, AL, Monday–Friday, 8am–5pm. The role requires at least 1 year of home health experience with OASIS; HCHB is preferred.

You will perform comprehensive patient assessments, collaborate with the care team, and ensure quality outcomes while maintaining timely documentation and discharge planning. The position offers competitive hourly pay and benefits through Amedisys.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• May be assigned RN visits as member of care team, and/or may be required to supervise a patient caseload, including LPNs and HHAs.
• Performs patient comprehensive assessments and collaborates with patient multidisciplinary care team to develop/modify and implement an individualized plan of care as per physician orders that ensures quality, proper discharge planning and achieves desired outcomes and goals.
• Facilitates interdisciplinary communication and follow up through care coordination on assigned patients and/or caseload of patients for duration they are on service.
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so.
• Participates in clinical outcomes monitoring, follow up and care center performance improvement initiatives.
• Initiates, plans and provides staff education and performs competency and skills validation for staff as needed.
• Utilizes accepted company teaching materials in patient/family education.
• Regularly communicates patient progress to the clinical manager.
• Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines, including OASIS assessments, SOC/admissions, recertifications, ABNs, subsequent visits, physician orders, care coordination, etc.
• Other duties as assigned.
