QuadMed is looking to hire a Registered Nurse for our Family Health Center in Yorktown, IN

GENERAL PURPOSE OF JOB

The Registered Nurse will provide general nursing care and leadership, working with physicians and multidisciplinary team members. This position involves nursing functions such as documenting vital signs and chief complaints, administering medications and injections, phlebotomy, patient triage, authorizing medication refills, patient education, with a focus on wellness and prevention.

KEY RESPONSIBILITIES

1. Perform duties in accordance with the Nursing Code of Ethics and other guiding principles.
2. Assess, plan, direct, and evaluate total nursing care based on patient’s age, physical, psychosocial, and cultural needs, following established standards, policies, and procedures.
3. Provide case management by advocating for patients and serving as a resource and contact person for patients and families.
4. Offer basic wellness and prevention education tailored to patients’ age and learning needs.
5. Collect assessment data, document subjective data (chief complaint and allergies), and objective data (vital signs, weight, etc.).
6. Review health history information, focusing on environmental, self-care, and discharge planning.
7. Administer medications/immunizations accurately; perform therapeutic treatments and health screenings as directed by the provider.
8. Perform lab/diagnostic tests, prepare specimens, test, record results, and report critical lab values immediately.
9. Prepare examination rooms with necessary equipment and supplies.
10. Assist providers with examinations and procedures.
11. Use universal precautions when obtaining specimens and delivering patient care, maintaining confidentiality and following HIPAA guidelines.
12. Complete all documentation accurately and timely for regulatory compliance, demonstrating proficiency in electronic medical records.
13. Participate in quality improvement, continuing education, and other patient care programs as required.

JOB REQUIREMENTS

Education:

• Graduate from an accredited nursing program; BSN preferred.

Experience:

• At least three (3) years of experience in a primary care setting, familiarity with electronic medical records, phlebotomy experience, outpatient and pediatric experience preferred.

Certificates, Licenses, Registrations:

• Valid nursing license in the state of the position or a compact state license.
• Current CPR (BLS) certification or must be obtained within 60 days of hire.
• PALS certification is a plus.

Knowledge, Skills & Abilities:

• Strong nursing, assessment, evaluation, diagnosis, and treatment planning skills.
• Excellent communication and organizational skills, ability to work independently.
• Proficiency in technology and a commitment to diversity and inclusion.
