Qualifications

Job description

Alliance Homecare VNA is seeking a dedicated and compassionate Registered Nurse (RN) to join our healthcare team in Woburn, Massachusetts.

The Registered Nurse will provide quality nursing care to patients while working collaboratively with patients, families, caregivers, physicians, and other members of the healthcare team.

Responsibilities

• Provide skilled nursing care in accordance with established care plans, physician orders, and applicable standards.
• Assess patients' health conditions and monitor changes in their clinical status.
• Administer medications and treatments as ordered and within the scope of practice.
• Perform nursing assessments and maintain accurate and timely clinical documentation.
• Communicate changes in patient condition and care needs to the appropriate members of the healthcare team.
• Educate patients and caregivers regarding medications, treatment plans, safety, and ongoing care needs.
• Coordinate patient care with physicians and other healthcare professionals.
• Follow organizational policies, clinical procedures, and applicable healthcare regulations.
• Maintain accurate patient records and documentation.
• Participate in care coordination and other clinical activities as assigned.

Qualifications

• Current and valid Massachusetts Registered Nurse (RN) license.
• Current CPR/BLS certification, as required.
• Previous nursing experience preferred; home health or community-based experience is a plus.
• Strong clinical assessment and communication skills.

Compensation & Benefits
