Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive in-home patient care. You will perform admission visits, assess eligibility for home health services, and develop individualized plans of care in collaboration with multidisciplinary teams.

The role requires RN licensure, a valid driver’s license, CPR certification, and reliable transportation. One year of RN experience is preferred, with opportunities for professional growth and access to ongoing

Competenze

• Graduate of an accredited diploma, associate or bachelor nursing program.
• Current Texas RN license.
• Current state driver's license.
• One year of RN experience preferred (two years).
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Mansioni

• Coordinate all aspects of the patient’s home care as the RN Case Manager.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
