Job summary

Angels Care Home Health in Scottsdale, AZ is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care and work with other medical disciplines to deliver professional care in the patient’s home.

The role includes performing admission visits, assessing eligibility for home care services, and developing the plan of care. A valid RN license in the state, CPR certification, and reliable transportation are required.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate nursing program.
• Current state RN license in the relevant state.
• Current state driver's license.
• One year of RN experience preferred (two years preferred).
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care as RN Case Manager.
• Interact with other medical disciplines to provide professional home care.
• Perform admission visits and develop the plan of care based on patient eligibility.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
