Job summary

Boost Home Healthcare is seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes on an intermittent basis. You will follow physician orders and an individualized care plan under guidance of the Client Services Director.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in the state of posting.
• Current BLS certification.

Responsibilities

• Make initial evaluation visits and reevaluate patient nursing needs.
• Meet daily productivity standards.
• Initiate and revise the plan of care as directed.
• Provide skilled nursing services requiring specialized expertise.
• Prepare clinical and progress notes after patient visits.

What You’ll Be Doing

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

• Currently licensed as a registered nurse in the state of the job posting.
• Must have current BLS Certification.
