We are looking for highly skilled Registered Nurses

Qualifications:

• Formal education as a Registered Nurse or Healthcare Aide or equivalent experience (RN, LVN, LPN)
• Strong verbal and communication skills in English
• In-depth understanding of disease processes related to vital sign monitoring (hypertension, heart failure, CKD, asthma/COPD, obesity)
• Time management and prioritization skills
• Assessment and critical thinking skills
• Clinical experience in inpatient settings is preferred
• Experience in adult care or pediatrics

Responsibilities:

• Conduct remote physiologic monitoring treatment management services, including interactive communication with patients/caregivers (99457)
• Call patients for verbal assessment and health education
• Respond to customer queries using computer/online guidelines
• Coordinate patient care and collaborate with healthcare team via electronic medical records
• Monitor vitals and assess if they are within or out of range
• Prioritize patient needs based on vital assessments
• Maintain patient confidentiality and respect rights in an online environment
• Monitor and evaluate patient progress, respond to alerts through remote monitoring tools
• Handle patient consultation calls
