Job description

We have multiple onsite positions throughout the US.

We are seeking a Registered Nurse to assess the health of individuals served and implement individualized care plans, with a focus on health maintenance, disease prevention, and case management. This role involves close collaboration with other health professionals and may include supervision of care plans and staff.

Key Responsibilities

  • Observe, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals in a community or small facility setting
  • Collaborate with other health professionals in planning, implementing, and evaluating programs and services addressing health needs
  • Coordinate communication and actions across disciplines related to health and safety
  • Ensure proper ordering, review, monitoring, discontinuation, and disposal of medications as prescribed
  • Help ensure policies are in place addressing medication administration, errors, storage/access, and destruction, and that these are reviewed and revised as needed
  • Provide progress reports to the physician when a client's condition changes
  • Transcribe or ensure transcription of physician phone orders and other medically related reports
  • Complete required nursing documentation on a daily, monthly, quarterly, and annual basis
  • Ensure routine documentation of individuals' health status in nurse's notes
  • Ensure furnishings, equipment, and supplies are maintained per infection prevention guidelines
  • Direct and supervise the activities of licensed practical/vocational nurses as needed
  • Ensure health-related appointments are scheduled and completed, with timely follow-up on findings
  • Maintain communication with inpatient nursing and discharge coordinators during hospitalizations
  • Provide new employee training and annual recertification training on health and safety topics
  • Ensure unlicensed staff are trained on person-specific health, safety, and disease prevention needs

  • Active RN license
