Position Overview: We are looking for a dedicated and compassionate Registered Nurse to join our team! This position focuses on supporting patients and their families, while also collaborating with the nursing team to provide timely, efficient care and ensure a positive, professional, and supportive patient experience.

Job Duties:

  1. Assist in the continuity of care by monitoring patient data.
  2. Facilitate communication with patients and provider offices.
  3. Build relationships with patients for service acceptance, device kitting, and fulfillment.
  4. Build relationships with partner physicians, ACOs, and hospitals to establish and adhere to continuum of care processes.
  5. Assist in creation and fulfillment of research and clinical grants for telemonitoring and in the creation/fulfillment of DSRIP initiatives with partner hospitals and communities.
  6. Provide behavioral health coaching.
  7. Assess patients’ formal and informal support systems, including caregiver resources and involvement, as well as available benefits and community resources.
  8. Evaluate patients’ progress toward goal achievement, identify barriers, and reassess care plans as needed.
  9. Research alternative treatment options.
  10. Communicate with supervising nurse, primary care physician, and care team members regarding patient status.
  11. Provide education, information, and support related to patient care goals.
  12. Act as patient advocate and assist with problem solving.
  13. Address barriers to patient care plans.
  14. Provide referrals to community resources.
  15. Facilitate access and communication among multiple services.
  16. Monitor activities to ensure services meet patient needs.
  17. Maintain accurate patient records and confidentiality.
  18. Measure outcomes and effectiveness of care management.
  19. Facilitate disease prevention and health promotion.
  20. Troubleshoot operational and clinical issues affecting patient outcomes.
  21. Make 100+ calls per shift to patients with critical readings.
  22. Monitor patient vitals and report concerns.
  23. Train clients on platform and processes.
  24. Other duties as assigned.

Job Requirements:

  • Active, unrestricted LVN, LPN, or RN license.
  • Graduate of an accredited LVN, LPN, or RN program.
  • At least 2 years of clinical experience.
  • Minimum upload/download speeds of 35/10 Mbps.
  • Ability to work from home in a HIPAA-compliant environment.
  • Proficient with EMR/EHR systems.
  • Understanding of clinical workflows, quality assurance, and compliance.
  • Excellent communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to manage multiple priorities.
