Job summary

Passion-Care is seeking a compassionate Registered Nurse (RN) to perform periodic check-ups for patients receiving home care. You will conduct routine health assessments, monitor chronic conditions, educate patients and caregivers, and collaborate with the care team to deliver high-quality, person-centered care.

Responsibilities include conducting wellness visits, updating care plans, documenting visits, and ensuring compliance with health regulations.

Qualifications

• Current and valid RN license in NJ.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Reliable transportation and valid driver’s license.
• Ability to work independently and manage time effectively.
• Familiarity with electronic medical records (EMR) systems preferred.
