Job summary

Amedisys is seeking a Registered Nurse Case Manager in Alabama to join a leading home health and hospice team. The role offers full-time hours, Monday through Friday, 8am–5pm, with on-call weekend rotation, empowering patients to heal at home.

Base salary ranges from $73,000 to $80,000 with conversion to a per-point pay model ($47–$53 per point) after orientation, along with comprehensive benefits, PTO, and a 401(k) plan. Join a trusted national network focused on quality patient care.

Qualifications

• 1+ year of clinical experience as an RN.
• Current RN license for the state(s) you work in.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note: if <6 months RN experience, may participate in RN Intern program.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through education and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.

Attractive pay

$73,000.00 to $80,000.00 Base Salary for Orientation and will then convert to a per point pay method with a rate of $47.00 to $53.00 per point

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
