Job summary

Amedisys is seeking a Registered Nurse Case Manager to join our home health team in Homewood, AL. This full-time role supports patients at home, collaborates with the care team, and develops care plans with on-call rotations on weekends.

Starting salary range is $73,000 to $80,000 with orientation pay, followed by a per-point pay method ($47–$53 per point). Benefit options include medical plans, PTO, 401(k) with company match, and wellness programs.

Qualifications

• One year of RN clinical experience.
• Current RN license for the assigned state(s).
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a care plan.
• Makes referrals to other disciplines as needed and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in clinical development and continuing education.

Attractive pay

$73,000.00 to $80,000.00 Base Salary for Orientation and will then convert to a per point pay method with a rate of $47.00 to $53.00 per point

• Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
