Registered Nurse -Home Health Broward County- Full time - Part time - PRN

The Field RegisteredNurse who is in the case manager of the Home Health Team is responsible for thenursing care of the patients assigned to them and directs the Home Health Aidesin quality patient care. The Field Registered Nurse is responsible for assessingpatient and family needs in order to promote the best care the Home Health cangive for recovery and rehabilitation. Provide nursing services within the scopeof practice authorized by the license issued by the State for a registerednurse.

Lines of authorityand reporting responsibilities: Report to the Director of Nursing, ClinicalManager, Administrator.

Job Functions:
  • Knows the philosophies,purposes, policies, and standards of the Home Health and their nursingservice department and provides for their explanation and implementationto the Home Health Aide. Be the case manager in all cases involvingnursing and therapy care.
  • Assesses in depth upon theadmission of the patient, the patient's physical and emotional status,level of competency, home environment, safety factors, family or householdmember's ability to assist with care and the need of the patient. Theseare incorporated into the admission notes. Conducts regular or OASISassessments accurately, according to the instructions in the OASISImplementation Manual, and corresponding to documents contained elsewherein the assessment note.
  • Help formulates a patientcare plan with the goals indicated and the means of implementing thecorrect procedures to attain these goals.
  • Records all clinical andprogress notes and enters them into the patient's permanent record files.Be responsible for the clinical record for each patient receiving nursecare.
  • Weekly reviews theutilization and progress of the patient with the supervisor and attendingphysician as necessary.
  • Has knowledge of patient'scondition at all times and informs the physician and/or the NursingSupervisor immediately of any change in the patient's condition thatwarrants attention. Also, observes, evaluates, and reports to thephysician the patient's reaction to drugs or treatments, or there aredeviations from the plan of care.
  • Interprets to the patient andfamily the expectations of the diagnosis and the nature of the treatmentconsistent with the action and wishes of the physician. Interprets to thesocial and physical factors in the environment that affect patient care.
  • Observe and evaluatespotential danger of disabling conditions and indicates preventive andcorrective measures.
  • Is responsible for theexecution of the physician's orders and keeps the physician informed ofall patient information concerning the patient's condition and response totreatment Gives skills or care to patients.
  • Extends paramedical servicesin carrying out the rehabilitative aspects of nursing care.
  • Obtains laboratory specimenswhen indicated per MD's orders.
  • Meets weekly with NursingSupervisor for the purpose of discussing nursing care, policies, and theHome Health Aides.
  • Assists the NursingSupervisor in surveying, analyzing, and determining staff requirements forher assigned patients.
  • Coordinates treatment withParamedical personnel.
  • Makes supervisory visits toall assigned Home Health Aides no less than once every two weeks.
  • Helps the family acceptresponsibility for providing care. Teaches and supervises family membersregarding care of the patient.
  • Assumes the responsibilityfor orientation of new personnel and participates in in-service trainingprograms.
  • Schedules her daily itineraryprimarily based the Priority of care needed, length of time visits willrequire, proximity to other patients to be visited and other relatedfactors.
  • A weekly itinerary is to beprojected for regularly scheduled visits, allowing time for newadmissions, emergency cases, and Home Health Aide introduction.
  • Must advise the office of anyitinerary changes and where she/he can be contacted at all times while inthe field. She/he should call the office between 9.00 to 5.00 p.m. eachday.
  • Responsible for thecertification and recertification of the Plan of Care.
  • May assign selected portionsof patient care to licensed practical nurses and home health aides, butalways retains the full responsibility for the care given and for makingsupervisory visits to the patient's home.
  • Performs other related dutiesas assigned by the Administrator.
  • Assure that progress reportsare mad to the physician for patients receiving nursing services when thepatient's condition changes or there are deviations from the Plan of Care.
  • Ensure HIPAA guidelines andprocedures are maintained. Participate in QAPI programs as needed.
PHYSICAL REQUIREMENTS:
  • Able to speak, read, andwrite in English
  • Able to read assignments,follow directions.
  • Able to communicate andrespond clearly on telephone and respond to patient's spoken needs.
  • The ability to transfer, liftor assist patients whose average weight is 160 pounds with or without theaid of mechanical devices.
  • Able to spend 80% of the workstanding and/or moving about.
  • Able to walk, climb stairs,stoop, twist, bend, and squat to perform essential job functions.
MENTAL REQUIREMENTS:
  • Able to concentrate on detailwith frequent interruptions.
  • Able to follow, complete andremember daily routines and requirements.
  • Able to comprehend andutilize professional education materials.
  • Able to cope with the mentaland emotional stress of the position.
QUALIFICATIONS:
  • A graduate of an accreditedSchool of Nursing and be licensed in the state of Florida.
  • Complete knowledge of nursingprinciples and procedures of skills in the technique of good patient care.
  • Good mental health and thequality of retaining emotional stability in situations of varyingcircumstance.
