Job summary

Tandemhealthsc is seeking a licensed practical nurse or registered nurse to administer skilled nursing care per the medical plan, assess patients, and support exams and procedures in our center. Responsibilities include vital signs, medications, wound care, and documentation in the EMR.

Ideal candidates hold SC licensure (RN or LPN), CPR certification, and have at least 2 years clinical experience. This on-site role emphasizes patient education, triage, and adherence to HIPAA and infection

Qualifications

• Graduate of accredited registered nursing program or licensed practical nursing program and maintains a current license to practice in South Carolina by the SC State Board of Nursing.
• Minimum 2 years clinical experience preferred.
• Current CPR certification required.

Responsibilities

• Administer skilled nursing care per the medical plan of care.
• Obtain vital signs and patient history.
• Prepare patients and assist with exams and procedures.
• Administer medications and immunizations.
• Collect specimens and perform point-of-care testing.
• Provide basic wound care and monitor patient status.
• Communicate results to patients as directed by the provider.
• Conduct follow-up calls and assist with patient and family education for continuation of care.
• Assist with precepting new clinical staff and student nurses.
• Document care in the electronic medical record accurately and timely.
• Manage nurse-directed patient calls and walk-in requests for healthcare services.
• Provide triage decision-making when evaluating patient needs using established protocols.
• Maintain HIPAA compliance.
• Follow infection control policies.
• Perform other clinical duties as assigned by the supervisor.

• Administer skilled nursing care to patients according to their physical and emotional needs and in accordance with the medical plan of care, including assessing, planning, implementing, and evaluating the care of assigned patients in the center, as appropriate.
• Obtain vital signs and patient history.
• Prepare patients and assist with exams and procedures.
• Administer medications and immunizations.
• Collect specimens and perform point-of-care testing.
• Provide basic wound care and monitor patient status.
• Communicate results to patients per provider direction.
• Perform follow-up calls and assist with patient and family education for continuation of care.
• Assist with precepting new clinical employees and student nurses.
• Document all care in the electronic medical record accurately and timely.
• Manage nurse-directed patient telephone calls and walk-in patients requesting healthcare services.
• Provide triage decision-making when evaluating and assessing patients' healthcare needs based on signs and symptoms, using established triage protocols and guidelines.
• Maintain HIPAA compliance.
• Follow infection control policies.
• Perform other clinical duties as assigned by the supervisor.
