Job summary

Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care to patients in their homes in Starkville, MS. The role requires coordinating with the interdisciplinary team and managing patient care plans.

Responsibilities include assessments, medication administration, family education, and on-call rotation, with opportunities for growth and continuing education. A BSN/CHPN is preferred, with CPR/BLS and a valid driver's license required.

Qualifications

• Registered Nurse (RN) licensed in the state of employment.
• BSN or Certified Hospice and Palliative Nurse (CHPN) preferred.
• Minimum 1 year of nursing experience in hospice, home health, or a related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to work independently and with a team.
• Pre-employment drug screening is required, including testing for marijuana.

• Supervise ancillary staff and ensure continuity of care.
