About the Role - We are currently seeking a Registered Nurse (RN) who is passionate about patient care and ready to make a meaningful impact. In this role, the RN will play a vital part in assessing patient needs, implementing care plans, providing education, and supporting recovery—all within the home setting. The nurse will be empowered to work independently while being backed by a strong, collaborative care team.

Responsibilities

• Assure quality care by adhering to standards set by the physicians
• Provide care education to patients in person or over the phone
• Adhere to compliance guidelines throughout processes (OSHA, FDA, HIPAA)
• Identify, assess, and monitor patients’ health conditions
• Administer medications and provide treatment according to physician’s orders
• Provide health education, medication review, healthcare coordination, and specialist referrals
• Monitor blood pressure and chronic conditions
• Ensure that all treatments and medications are fully documented in patients’ health records

Required skills and qualifications

• Current and unrestricted RN license in the state of Illinois
• Certification in CPR required
• Working knowledge of OSHA, FDA, and HIPAA compliance standards
• Ability to lift, move, and transport patients safely
• Strong communication, time management, and critical thinking skills
• Prior experience in home health or a similar setting is preferred

Pay range and compensation package - Starting at $72,800
