Responsibilities of a Hospice RN / RN Case Manager include:
- Reporting directly to the Administrator, Executive Director, or Patient Care Manager and maintaining primary control and professional management of each patient.
- Acting as the primary liaison between the physician, patient/family, and hospice team.
- Providing direct patient care services to hospice patients and supportive care to the patient’s family with a manageable caseload, fostering a trusting and compassionate relationship.
- Empowering patient’s caregivers through education on care for their loved one.
- Managing and collaborating with an interdisciplinary team including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
- Evaluating patient and family needs and assisting in the development and execution of the plan of care for each patient.
- Maintaining all assigned patient clinical records.
- Participating in company quality assessment and performance improvement programs.
About You
Requirements for Hospice Registered Nurse / RN Case Manager:
- Compassionate, kind, and empathetic attitude towards patients and their families.
- Comfortable providing care in the patient's home environment.
- Flexibility and composure to support others during crises.
- RN license in the state of employment or eligibility to obtain one.
- At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, critical care, etc., is advantageous.
- Current driver’s license and automobile insurance.
- Current CPR certification.
