Job summary

La Vita Bella Home Health and Hospice seeks an experienced registered nurse to provide one-on-one home health care, building meaningful relationships with patients and families. You’ll support healing in a home setting with autonomy and a collaborative team.

Responsibilities include patient assessments, vital signs, medication review, and educating families on care strategies. Travel to patients' homes is required, with a competitive pay structure and mileage reimbursement.

Qualifications

• 18 years of age or older.
• Ability to pass a criminal background check.
• Valid driver’s license and insurance on personal vehicle.
• Ability to lift 50 pounds.
• Willingness to travel to patients' homes.

Responsibilities

• Assess patient’s condition during every visit and chart observations.
• Perform evaluation tasks, including vital signs and medication review.
• Educate patients and their families on home health care strategies and procedures.
• Coordinate with the patient's care team, including community providers.
• Make recommendations for devices or tools to help improve patient’s quality of life.
• Listen to the patient and respond to concerns or requests.
