Job summary

La Vita Bella Home Health seeks a compassionate, experienced RN to provide one-on-one home care, assess conditions, chart observations, and educate patients and families in a supportive setting.

You’ll coordinate with the care team, travel to patients’ homes, and enjoy flexible scheduling, competitive pay, and growth opportunities through CEUs and advancement.

Qualifications

• Active state RN license is required.
• Ability to pass a criminal background check.
• Valid driver's license and personal vehicle insurance.
• Ability to lift up to 50 pounds.

Responsibilities

• Assess patient's condition during visits and chart observations.
• Perform evaluation tasks, including vital signs and medication review.
• Educate patients and families on home health care strategies & procedures.
• Coordinate with the care team and community providers.
• Recommend devices or tools to improve quality of life.
• Listen to patient concerns and respond to requests.
