Job summary

OPCO Skilled Management seeks a Charge Nurse (RN or LVN) for our Los Alamos, NM facility in a full-time capacity. The role emphasizes delivering nursing care and supervising staff within regulatory standards.

The RN/LVN will develop care plans, monitor safety, and collaborate with physicians, therapists, and nurses to meet resident needs. A valid license and BLS/CPR are required.

Qualifications

• Hold current LVN/LPN or RN license in practicing state.
• Current BLS/CPR certification or willingness to obtain one.
• Experience in nursing management or supervisory role is preferred.

Responsibilities

• Follow established charting procedures and incident reporting.
• Oversee narcotic counting, pharmacy orders, and restraint reduction per policy.
• Document patient condition changes, including nutrition, infections, skin conditions, or weight changes.
• Utilize the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Document and notify departments of room changes, LOA, new admits, or discharges.
• Round with physicians as needed to ensure patient care continuity.

The Registered Nurse or Licensed Vocational Nurse will be responsible for resident care and directing nursing staff. The RN is expected to develop the plan of care and collaborate with physicians, therapists and nursing assistants. The RN will also monitor assigned staff to ensure that all safety regulations are upheld.

Charge Nurse (RN or LVN) will provide nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

Qualifications

• Current LVN/LPN or RN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Responsibilities

• Documents any change in patient’s conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient’s room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
