Job summary

Ambercare Homecare in Santa Fe, NM seeks a compassionate Registered Nurse (RN) for full-time home health roles. New graduates are welcome to apply, with responsibilities across assessment, care planning, and coordination with a multidisciplinary team.

The RN will perform nursing procedures, update care plans, and support patients and families with clinical needs, while maintaining accurate records and ensuring quality care in the home setting.

Qualifications

• Graduate from an accredited RN program and licensed in the state.
• At least one year clinical nursing experience, preferably ICU, rehabilitation, or medical-surgical.
• Home health experience preferred.
• CPR certification required; reliable transportation, valid driver's license and auto insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, admission processing, planning, implementation and evaluation of the nursing process.
• Assess data from patient physical, psychological, social and spiritual factors that may impact patient and family needs and coordinate intervention by the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team and other agencies for coordinating optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the physician.
• Perform clinical pronouncement of death and provide family support as applicable.
• Maintain accurate patient records, charts, progress notes, and timely care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
