Position Overview

We are hiring an experienced and compassionate Registered Nurse (RN) to deliver skilled nursing services to patients within their homes. You will develop individualized care plans, promote patient health education, and coordinate with physicians and interdisciplinary teams to ensure the highest quality of care.

Key Responsibilities
  • Conduct comprehensive assessments and document findings.
  • Initiate and modify individualized care plans based on ongoing assessments.
  • Perform skilled nursing procedures, administer medications, and provide wound care.
  • Educate patients and families to support self-care and improved health outcomes.
  • Communicate effectively with the healthcare team and patient's physician.
  • Participate in scheduled on-call rotations.
  • Supervise Home Health Aide services, ensuring quality and compliance.
Qualifications
  • Graduate of an accredited nursing program.
  • Active RN license in California and BLS certification.
  • Reliable transportation, driver’s license, and insurance.
  • Minimum of one (1) year of acute or home health experience preferred.
  • Bachelor’s degree preferred.
Physical Requirements
  • Frequent standing, walking, bending, lifting up to 50 lbs.
  • Ability to travel throughout San Diego County to provide patient care.
