Job summary

Compass Connections is seeking a compassionate RN to join our team at our facility in Texas. You will provide triage, administer medicines, and coordinate care for pediatric patients in our shelter and group home environment.

The role requires an active Texas RN license, 1+ year of RN experience (pediatric nursing preferred), and compliance with HIPAA and safety protocols. BLS is required; PALS and bilingual (English/Spanish) are preferred.

Qualifications

  • Must hold a Registered Nurse license in Texas.
  • Minimum 1 year RN experience; pediatric nursing preferred.
  • Current BLS; PALS preferred, First Aid/ CPR; Emergency behavior intervention beneficial.

Responsibilities

  • Provide triage services to assess patient needs and determine urgency.
  • Document medical information accurately in EMR and other records.
  • Administer medications and monitor patient responses as prescribed.
  • Coordinate health services for children in care, including preventive care.

Hourly Rate: $43.86 an hour.

Language Requirement: Fluency in English and Spanish is required.

Work Schedule: 7pm-8am (3days) and 7pm-11pm (1day)

Academic Req : Required - Registered nurse license; Preferred - Bachelor's of Science in Nursing

Certifications : Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention

Work experience: Required - At least one year of experience as a RN; Preferred - pediatric nursing experience

Critical Action Items & Measurable Deliverables:
  1. 1. Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  2. 2. Provide triage services to assess patient needs and evaluate symptoms. This includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
  3. 3. Have extensive knowledge of medical documentation to be able to generate, decipher, and correct for proper documentation methods in accordance with state and federal guidelines.
  4. 4. Perform nursing duties and tasks relevant to this position in compliance with all regulatory authorities and program protocols including, but not limited to, active and ongoing nursing assessments, charting, scheduling appointments, routine observations, isolation observations, wellness checks, immunizations, medication administration, and time internal reports, when needed.
  5. 5. Ensure patient safety by assessing and monitoring health conditions, assist with procedures and operate medical equipment, administer vaccines, medications and treatments ordered by healthcare provider, monitor for side effects and reactions, and provide teaching to patients and staff.
  6. 6. Arrange and coordinate appropriate health services, including urgent, emergent, and preventative health services for children in care.
  7. 7. Conduct health assessments based on established protocols and accurately document medical findings, including validating systems of record for required facility documentation and reporting to meet agency, federal, and state guidelines within required timeframes.
  8. 8. Supervise, direct, and delegate when appropriate to execute medical services reflected in healthcare plans and treatment orders, including medication administration, to ensure that all treatment orders directed by the prescribing healthcare professional are carried out as ordered and monitor the patient for improvement.
  9. 9. Gather, collect, and clarify patients' current condition and past medical history.
  10. 10. Communicate with pharmacies on ordering prescription medications, check medication deliveries for accuracy and compliance with orders and remediate any discrepancies.
  11. 11. Maintain adequate over the counter medication supplies for each service population facility and arrange proper disposal of unused and expired medications.
  12. 12. Maintain documentation of controlled drugs that may be available to include up to the date reconciliation of all scheduled drugs used in the facility through the facility's protocols and procedures.
  13. 13. Administer medication as ordered by preserving healthcare provider and complete accurate and timely documentation in electronic medical records and Medication Administration Record.
  14. 14. Effectively communicate with Certified Nurse Practitioner and Lead Medical Coordinator as needed for reporting findings concerning for patient's health and well-being.
  15. 15. Communicate with Lead Medical Coordinator and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.
  16. 16. Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
  17. 17. Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
