Responsibilities of a Hospice RN/ RN Case Manager include:

• Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
• Acting as the primary liaison between the physician, patient/family, and hospice team.
• Providing direct patient care services to hospice patients and supportive care to the patient’s family, managing a reasonable caseload while building trusting and compassionate relationships.
• Empowering patient’s caregivers through education on care for their loved ones.
• Managing and collaborating with an interdisciplinary team including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
• Evaluating patient and family needs and assisting in the development and execution of the plan of care for each patient.
• Maintaining all assigned patient clinical records.
• Participating in company quality assessment and performance improvement programs.

About You

Requirements for Hospice Registered Nurses include:

• A compassionate, kind, and empathetic heart for patients and their families.
• Comfort with providing care in the patient's home environment.
• Flexibility and composure to support others during crises.
• RN license in the state of employment (or eligibility to obtain).
• At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
• Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, critical care, or wound care is advantageous.
• Current driver’s license and automobile insurance.
• Current CPR certification.
