Job summary

ComForCare Home Care - Reno is seeking a Registered Nurse to perform in-home client assessments, develop personalized care plans, and orient caregivers to each client’s needs. Based in Reno, you will help clients transition safely home from hospital or other care settings.

You must be a graduate of an accredited nursing program with an unrestricted Nevada RN license and CPR. The role offers a clinician-led environment, flexible scheduling, and opportunities for advancement, with pay-per-visit

Qualifications

• Graduate of an accredited school of professional nursing.
• Current, unrestricted Nevada RN license.
• CPR certification.
• Demonstrated knowledge of safety and infection control practices.
• Home health, case management, or hospital discharge planning experience preferred.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Perform comprehensive in-home assessments of client status, including physical, psychosocial, and environmental factors.
• Develop an individualized care plan for each client based on assessment findings and client goals.
• Orient caregivers to each client’s routines, safety needs, and care plan.
• Attend hospital and facility discharge meetings to ensure follow-up needs are captured in the care plan.
• Reassess clients and updating care plans as needs change.
• Maintaining accurate, timely client documentation.
• Identifying skilled care needs and coordinating referrals to home health, physicians, or other providers.
• Monitoring client progress, adjust care plans as needed, and communicate updates to physicians and care team members

Compensation:

This is a flexible, pay-per-visit position: you’re paid a flat rate for each completed visit, plus mileage, rather than an hourly wage.

• $150 per in-home assessment (includes care plan and caregiver orientation)


• $200 when a hospital or facility discharge meeting is included


• $100 per combined reassessment and supervisory visit


• Mileage reimbursed beyond 20 miles round trip

Visits throughout Reno, Sparks, and surrounding areas.
