Job summary

Penn Medicine at Home in Monroe and Jamesburg, NJ is seeking a compassionate Registered Nurse (RN) to join our Home Health team. You will deliver skilled nursing care in patients' homes, build meaningful patient relationships, and contribute to high-quality, personalized care in the comfort of home.

You'll collaborate with physicians, therapists, and case managers, enjoy clinical autonomy within a world-class health system, and engage in ongoing education and career development opportunities.

Qualifications

• Active RN license.
• Experience in Home Health, Med-Surg, Acute Care, or Community Health preferred.
• BLS/CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver skilled nursing care to patients in their homes.
• Conduct assessments and develop individualized care plans.
• Educate patients and families on disease management, medications, and preventive care.
• Coordinate care with physicians, therapists, case managers, and interdisciplinary team members.
• Support patients throughout their recovery journey while promoting independence and wellness.
