Job summary

Hill Valley Healthcare in Indiana is seeking Registered Nurses to provide compassionate, resident-centered care in a skilled nursing and long-term care setting.

You’ll administer medications, plan and document care, educate families, collaborate with therapists, and help maintain safe staffing ratios; a 10k sign-on bonus is available for full-time hires.

Qualifications

  • Active state RN license required.
  • CPR/BLS certification required.
  • Experience in Long-Term Care or Skilled Nursing preferred.
  • Excellent technical, assessment and documentation skills.
  • Strong analytical and problem-solving abilities.
  • Ability to lift 25 pounds or more and stand/walk for extended periods.
  • Team player with strong communication and compassion for seniors.

Responsibilities

  • Provide nursing care to meet resident needs following standards and orders.
  • Plan nursing interventions and monitor implementation for each resident.
  • Evaluate care effectiveness, identify problems, and develop alternatives.
  • Conduct family and resident education.
  • Administer medications and treatments per policy and orders.
  • Transcribe orders and document progress with resident responses.
  • Report changes in condition to supervisor, physician, and family.
  • Monitor skin changes and follow skin management protocols.
  • Coordinate rehabilitation programs with therapists.
  • Ensure CNA documentation and assignments are completed on time.

What You’ll Be Doing:
  • Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

  • Plan nursing interventions for each resident based on individual needs and monitor the implementation

  • Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.

  • Conduct family and resident education

  • Administer medications and treatments timely and according to center policy

  • Transcribe physician’s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response

  • Identify and report changes in condition to supervisor, physician and family

  • Accurately identify skin changes and follow skin management protocols

  • Order and report diagnostic tests timely

  • Follow Pharmacy policy and procedures for ordering and delivering medications.

  • Coordinate resident’s rehabilitation program with therapists to ensure rehabilitation needs are met

  • Monitors that CNA documentation is completed daily

  • Ensures CNA assignments are completed on time

  • Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.

Requirements:
  • Active state RN license

  • CPR/BLS Certification

  • Experience in Long Term Care or Skilled Nursing preferred

  • Excellent technical, assessment and documentation skills

  • Strong analytical and problem-solving skills

  • Ability to lift 25 or more pounds

  • Ability to stand, bend, and walk for extended time periods

  • Team player, strong communicator, passionate about senior care

Compensation for the role will depend on several factors, including a candidate’s qualifications, skills, competencies and experience, and may fall outside of the range shown.
