Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate home health services and interact with multiple medical disciplines to deliver quality patient care. The RN will conduct admission visits in patients’ homes, determine eligibility for home care services, and develop individualized care plans.

Qualified candidates hold a state RN license, CPR/ Hepatitis consent, and a valid driver's license. One year of RN experience is preferred.

Qualifications

• License as a Registered Nurse in the state.
• 1 year RN experience preferred (2 years preferred).
• CPR certification and Hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of home health patient care.
• Interact with other medical disciplines as needed to provide professional care.
• Perform admission visits, determine eligibility, and develop the plan of care.
