Job summary

Angels Care Home Health in Lamesa, Texas is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care. You will interact with other medical disciplines as needed & provide professional care to the patient.

You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed. Requirements include graduated from accredited nursing program with RN license, current Texas

• Graduate of an accredited nursing program with RN licensure.
• Current CPR certification and Hepatitis consent/declination.
• Valid driver's license and reliable transportation.
