Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate home care for patients in their homes. You will conduct admission visits, assess eligibility, and collaborate with multidisciplinary teams to develop and monitor individualized care plans.

Qualifications

• Graduate of an accredited nursing program (diploma, ADN, or BSN).
• Current RN license in the state.
• Valid driver’s license and reliable transportation.
• CPR certification; Hepatitis consent/declination on file.

Responsibilities

• Coordinate all aspects of the patient’s home care and services.
• Conduct admission visits and determine eligibility for home care.
• Develop and monitor the patient’s plan of care with the care team.

You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed.
