Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate all aspects of home care for patients. Responsibilities include admission visits, eligibility assessment, care plan development, and collaboration with the medical team to deliver quality care in the patient’s home.

Qualified candidates hold an RN license, nursing degree, and a valid driver’s license; CPR and Hepatitis consent/declination are required.

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license.
• Valid driver's license.
• One year as RN; two years preferred.
• CPR certification and Hepatitis vaccination/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to determine eligibility for home care services.
• Develop and implement plan of care in collaboration with medical team.
• Interact with other disciplines to ensure patient needs are met.
