Job summary

Neighborhood Adult Day Care Center seeks a compassionate Registered Nurse (RN) to join our healthcare team and provide high-quality nursing care to participants during daytime hours.

The RN will assess health needs, coordinate with families and physicians, administer medications, monitor conditions, and contribute to care planning and safety in a supportive, community-based environment.

Responsibilities

• Health Assessments: Conduct initial and ongoing health assessments of participants.
• Medication Management: Administer medications as prescribed, educate clients and caregivers on medication use, maintain accurate medication records.
• Wound Care & Chronic Disease Management: Provide basic wound care, monitor/ manage conditions like diabetes, hypertension, dementia.
• Emergency Response: Respond to medical emergencies and coordinate with EMS or hospitals as needed.
• Develop Care Plans: Collaborate with staff to create individualized care plans.
• Communication: Serve as liaison between participants, families, and healthcare providers; provide updates to families.
• Documentation: Maintain nursing notes and health records; document changes in condition and outcomes.
• Health Education: Teach healthy lifestyle habits; provide caregiver training.
• Promote Safety & Hygiene: Oversee infection control and ensure a safe environment.

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our healthcare team. The RN will be responsible for delivering high-quality nursing care to patients, coordinating with family members and physicians, and ensuring adherence to established policies, procedures, and safety standards.

Core Clinical Responsibilities

1. Health Assessments:

2. Medication Management:

   • Administer medications as prescribed.

   • Maintain accurate medication records.

   • Collaborate with other staff (social workers, physical therapists, etc.) to create individualized care plans.

2. Communication:

   • Serve as liaison between participants, families, and healthcare providers.

3. Documentation:

   • Maintain up-to-date nursing notes and health records.

   • Document changes in condition, interventions, and outcomes.

Supportive & Educational Duties

1. Health Education:

   • Teach participants about healthy lifestyle habits, disease prevention, and self-care.

2. Promote Safety & Hygiene:
