Job summary

Sevita is seeking a Registered Nurse to provide care and treatment for individuals in an intensive residential treatment setting who have significant psychiatric needs. The role requires strong collaboration with a multidisciplinary team and adherence to trauma-informed, person-centered care principles.

Responsibilities include medication management, care planning, documentation, and counseling activities. Travel may be required; strong communication and organizational skills are essential.

Qualifications

• Must be 21 years of age.
• Bachelor's degree in nursing or related health field with at least 2000 hours of experience with adults with serious psychiatric disability.
• RN from an accredited program with more than 6,000 hours of experience with adults with serious psychiatric disability.
• Valid driver’s license and insurance.
• Ability to communicate verbally and in writing.
• Experience in managing systems, processes, and people.
• Ability to work independently as well as part of a team.

Responsibilities

• Attends and actively participates in weekly team meetings.
• Provides rehabilitation and coordinates medical needs for clients.
• Medication monitoring and administration (oral or injections).
• Group and individual counseling and case management.
• Writes progress notes, summaries, and discharges.
• Promotes consumer health and safety and follows health plans.
• Ensures compliance with contractual, legal, and regulatory requirements.

Summary

The Registered Nurse is responsible for providing care and treatment for people served in an intensive residential treatment setting that are experiencing significant mental health/psychiatric symptoms, who are demonstrating significant functional impairments, and who may have co-occurring substance abuse and/or personality disorder.

Essential Job Functions

• Attends and Actively participates in weekly team meetings
• Provides rehabilitation and coordinates medical needs for clients.
• Medication Monitoring and Administration by oral medication or needle injections
• Medication training for staff
• Orders, reviews, monitors, and discontinues medications as ordered by the physician
• Assist with keeping within assigned nursing budget.
• Conducting assessments
• Approving plan of care and service delivery methods
• Performs competency and skills evaluations on employees who provide direct care.
• Informing the physician and staff of changes in the client’s medical or mental condition and needs
• Coordinates psychiatric information with the psychiatric rehabilitation practitioners.
• Writes Progress notes, summaries, and discharges

• Implements the treatment/rehabilitative activities and strategies
• Crisis Prevention
• Reports significant / major incidents or accidents.
• Provides and arranges necessary medical services to ensure consumer health and safety. Follows consumer health plans.
• Provides and promotes consumer choice and facilitate independence.
• Assist with ensure compliance with contractual, legal and regulatory requirements.

Qualifications/Education

• * Must be 21 years of age
• . Holds a bachelor's degree in nursing or related health and human services field from an accredited college or university with at least 2000 hours of experience with adult persons who have a serious psychiatric disability or
• OR Holds an RN from an accredited two year program and has more than 6,000 hours of experience with adult persons who have a serious psychiatric disability.
• Valid driver’s license and insurance required
• Ability to communicate verbally and in writing.
• Experience in managing systems, processes, and people.
• Operate general office equipment
• Good organizational skills and attention to detail
• Capable of working responsibly with highly confidential information
• Must be able to work independently as well as part of a team

Work Environment And Physical Demands

• Physical demands: Must be able to utilize proper body mechanics while lifting up to 50 pounds, must be able to bend, stoop, push, pull reach, sit, and walk for periods of time. Must have fine vision, sustained vision, and peripheral vision. Must possess sufficient eye/hand coordination to operate office, adaptive, and household equipment.

Work environment: Light and ventilation as found in typical home or office setting. Hazards may exist if the employee fails to use proper body mechanics when lifting or transporting an individual or item, if vehicles are not operated safely, or if proper techniques are not used when intervening with individuals. The possibility of blood borne pathogens exists if universal precautions are not followed.
