Role Responsibilities

• Coordinate total care plans with healthcare professionals, patients, families, and community resources within a Point of Care setting.
• Conduct initial and comprehensive nursing evaluations, develop plans of care, and ensure proper documentation in collaboration with physicians.
• Perform patient assessments and develop appropriate care plans at designated times.
• Verify patients meet home health eligibility and medical necessity guidelines as defined by payers.

License and Certification Requirements

• Current RN license in the practice state
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
